Scanning Information Checklist If you wish to order a digital scan use this form to indicate what type and quality of scan is desired. Reproduction Fee Schedule This form gives a list of prices charged for reproductions of materials in the Multimedia Archives.Permission to Publish Form This form is a contract with the J. Willard Marriott Library, University of Utah, for the use of Multimedia Archives images in a publication, advertisement, exhibit, display, etc. This form is accompanied by a "Conditions of Use" statement. Use Fee Schedule If you wish to use an image from the Multimedia Archives in a commercial publication, advertisement, exhibit, display, etc., use fees are required. This form is a list of fees charged for the commercial use of Multimedia Archives materials. Deed of Gift This is the standard gift instrument used for donating materials to the J.
